5 - PROGRAM / SERVICE / ACTIVITY PURPOSES
General
To promote a social model complementary to the residential model for attending older people living alone, at the same time realistic with respect to the ageing process, emphasizing the importance of early prevention, intergenerational approach and a more active.

Specific
- To improve the quality of life and well-being of older people with special needs and people who live alone.
- To achieve older people’s personal self-esteem.
- To demonstrate the efficacy and effectiveness of the attention to older people in their own territory compared to other kind of attention in institutions.
- To demonstrate higher level of satisfaction in older people ageing in their own village, at their home, than those who do it in institutions, provided that they are attended by people who care for them, as professionals from their own area can be, and who do it properly.

The program was implemented by the KMOP, in eight municipalities of Greece. Each structure employed at least one nurse, a graduate of social studies and a household assistant. The executives were obliged to visit at least once a week all beneficiaries in their home in order to serve them according to their needs (nursing care, personal care, home care, psychological support, external works, etc.)

8 - PROGRAM / SERVICE / ACTIVITY SUMMARY DESCRIPTION
The program "Help at Home" is a program that supports the social dimensions of care and service for elderly persons in need of home care. It is committed to building services, through which is promoted the employment or reintegration of the unemployed into the labor market, especially women, with the high participation of those in specialties requiring created services (social workers, psychologists, nurses and family helpers).

Personal service to help and to attempt to address personal or family problems of the target group and their families. To this end we work together in relevant scientific disciplines (psychiatrists, psychologists, social workers, etc.) and Principles or public agencies, and the private sector, dealing with this problem and authority (formal or substantive) for the solution or remedy it. It also includes:

- Psychological support at individual level. These frameworks would provide assistance to enhance self-esteem, anxiety, internal conflicts and potential negative reactions from various difficulties.
- Tracking individual cases to identify and prevent any problems.
- Exercise counseling, discussion, psycho-emotional support, facilitate in finding solutions to their problems and a general activation of the case.
- Domiciliary visits for conducting social research and depending on the specificity and importance of the problem to the continuous and often lengthy contact and communication with the people.
- Monitoring of specific cases of people facing serious integration problems in social or other difficulties, together with their families.
